In compliance with local government guidelines, this permit serves as the official safety and regulatory clearance required to authorize a Change of Name for the account.

This is a signed authorization form or letter from the registered account owner, explicitly granting permission to transfer the electric service account to your name. This ensures that the account transfer is fully authorized by both parties
